Position Summary
The Youth and Young Adult (YYA) Case Manager Team Lead (CMTL) is responsible for providing comprehensive case management to young adults (age 18-24) at the Men’s Shelter and other emergency shelters in the Continuum of Care as needed. This includes engagement, linkage to behavioral health and medical services, housing navigation, and income enhancement. The (CMTL) helps homeless young adults achieve housing stability by assisting them in transitioning to permanent housing and increasing their income for rapid exit. The goal is to ensure homelessness is brief, rare, and a one-time experience through high-quality and responsive case management.
Some Of The Duties Include
Engage and provide case management services to (YYA) clients at the Men’s Shelter and other emergency Shelters
Support clients in creating Exit Plans with Shelter staff to ensure rapid exit from homelessness and linkage to appropriate destinations
Link and refer clients to resources and community partners for identification, benefits, mental health, substance use disorder services, and housing interventions
Assist clients in increasing their incomes through job training programs, employment opportunities, or SSI/SSDI applications
Ensure clients are linked to Cuyahoga County’s Coordinated Entry if not assessed prior to Shelter entry
Assist in arranging transportation to critical destinations (e.g. housing, medical, employment)
Maintain participant data and statistical information, client database records, Housing Assessments, and progress notes in HOPE and User Form data tracking systems
Maintain regular contact with partner agency staff for individual client updates
Collaborate with the Clinical team regarding mental health and substance use crises
Participate in regular caseload reviews with supervisors
Assist the supervisor in leading team initiatives, facilitating staff meetings, representing YYA Case Management in the community, and making decisions in consultation with the supervisor
Create and maintain relationships with health providers and community partners
Provide crisis intervention for clients as needed
Implement wrap-around case management services
Qualifications
Our ideal candidate has a High School Diploma or GED; Bachelor’s Degree a plus. Experience identifying, accessing, and coordinating with community partners and social service resources ideal; experience working with vulnerable populations and people with lived experience; and the ability to uphold and support the Housing First Philosophy. A valid driver’s license with proof of insurance required.
