The Medication Safety Coordinator shall work with the Pharmacy team and shall be responsible for ensuring the department and organization follows all aspects of medication management, ensuring medication safety in compliance with all regulatory laws, statues and accreditation organization initiatives. Current practices shall be continually evaluated and monitored to ensure optimal practice. He/she shall lead changes when appropriate. He/she shall be well versed in evidence-based literature or from own data striving for best practice and patient safety. He/she shall ensure that all policies and procedures are updated to support these practices teaching these policies to staff members. He/she shall coordinate education/training. Will interact with other hospital disciplines including the medical staff to effect positive change. The Medication Safety Coordinator is an advocate for the patient. He/she will be a member of the Medication Safety Sub-Committee and will report to the Director of Pharmacy and Pharmacy Operations Manager.

Required Skills
Possess basic knowledge in pharmacokinetics with the ability to apply them in the hospital setting.
Possess the ability to critically think and have excellent organizational skills.
Possess knowledge of the Institute of Safe Medication Practice (ISMP) guidelines.
Possess knowledge of policies governing USP , USP, Medication Standards
Possess excellent computer, verbal/written communications, and the ability to prioritize.
Required Experience
Graduate of an accredited school of pharmacy recognized by the California State Board of Pharmacy
Possess a Pharm D degree and be California Registered Pharmacist in good standing.
Must have a current BLS/ACLS Healthcare Provider CPR card from the American Heart Association.
Minimum of five years hospital experience.
Knowledge of local, state and federal laws and regulations governing pharmacy practice, as well as other regulatory agencies (The Joint Commission, MERP, CDPH, CA BOP, CMS, DEA).
Preferred:
Completion of a hospital residency and/or specialty in compliance areas such as USP Chapters.
