Job Description:
Develop and implement individualized care plans for patients
Coordinate healthcare services and resources to optimize patient outcomes
Conduct assessments and collaborate with interdisciplinary teams

Education And Experience Requirements:
Active Registered Nurse (RN) license in the state of New Mexico
Minimum of 1 year of experience in case management
