Insufficient systems thinking during the project scoping period is of the primary ways a project can fall behind. Over time, even in the most effectively scoped projects, additional work or scope gaps appear, change management is involved, and issues can arise. The best way to counter this is to check your defn of the system(s) in place, how they are target orientated, versus what those targets are, and whether or not you are achieving those targets, or you're even measuring them? Efficient data tracking and measurement are the holy grail of successful projects. When it's not going right, examine the basics. It may be a bit painful for an ego or two when failings are identified, but it will save the critical path in the long run.
